What is HIV?
HIV is a virus—a submicroscopic infectious agent that can only replicate inside a living cell. In this case, HIV targets the cells of the immune system, particularly CD4+ T-cells, which are central orchestrators in the body’s defense against infection. When HIV enters these cells, it hijacks their machinery, forcing them to produce more viral copies. Diagnosis of HIV

Screening recommendations vary by country and risk profile, but most public health agencies now advocate for at least one routine HIV test for all adults. For those at higher risk—due to sexual exposures, injection drug use, or occupational exposure—more frequent testing is recommended. Early detection is key, not only because it allows individuals to start life-saving treatment sooner but also because it dramatically reduces the risk of onward transmission. Treatment suppresses the virus to undetectable levels, and the landmark principle of “U=U” (Undetectable = Untransmittable) means people with HIV who have an undetectable viral load cannot sexually transmit the virus.
Modern Treatment and Hope for the Future
The management of HIV has seen remarkable advances over the years, transforming a feared death sentence into a manageable chronic condition. Modern treatment centers on combination antiretroviral therapy (cART or simply ART), typically a once-daily oral regimen involving drugs from at least two classes—integrase inhibitors, nucleoside/nucleotide reverse transcriptase inhibitors, and non-nucleoside reverse transcriptase inhibitors, among others. These combinations work by blocking critical steps in the viral life cycle, suppressing viral replication to undetectable levels in the blood.
With consistent use of ART, individuals living with HIV can expect to live lives nearly as long as their uninfected peers. ART also prevents mother-to-child transmission and has transformed outcomes in children, especially in high-prevalence regions. According to UNAIDS, since HIV/AIDS was first identified, over 40 million people have died from AIDS-related diseases, but more than 28 million are currently alive and thriving due to treatment. Prevention strategies for HIV infection fall under a simple premise: reduce opportunities for the virus to enter the body and establish infection. This involves consistent use of condoms during sexual activity, regular and accessible testing, sterile needle exchange programs, and health education tailored to vulnerable communities. Pre-exposure prophylaxis, a daily medication for at-risk HIV-negative individuals, reduces the risk of acquiring HIV by up to 99% when taken as prescribed. Post-exposure prophylaxis (PEP), taken within 72 hours of exposure, can also be highly effective.
There’s also growing momentum around the concept of “treatment as prevention.” When a person living with HIV is stably on ART and maintains an undetectable viral load, transmission to sexual partners is effectively zero—a groundbreaking realization that has spurred public health campaigns like U=U worldwide. Mother-to-child transmission, once a tragic inevitability, can now be almost completely prevented if the mother has access to care.
Myths and Stigma Surrounding HIV/AIDS
Despite overwhelming evidence, myths and misconceptions about HIV/AIDS persist. Some still believe, wrongly, that HIV can be caught by hugging, sharing food, or using the same restroom as someone with the virus. These misunderstandings breed stigma, isolate individuals, and discourage them from seeking testing and care. Stigma is now recognized by UNAIDS as one of the biggest barriers to ending the epidemic, as social exclusion and discrimination can be even more devastating than the infection itself.
The lived experience of someone diagnosed with HIV has changed profoundly since the early days of the epidemic, but the psychological burden often remains. HIV/AIDS in Global Context
There is no single global narrative for HIV/AIDS—each region faces unique challenges shaped by culture, economics, and politics. Sub-Saharan Africa, for instance, accounts for nearly two-thirds of all people living with HIV worldwide. Here, women and girls are especially vulnerable; in some countries, adolescent girls represent a disproportionate share of new infections. Eastern Europe and Central Asia are seeing rapidly accelerating epidemics, driven by injection drug use and limited harm-reduction infrastructure.
At the same time, dramatic progress is possible. Some high-burden countries, such as Botswana and Rwanda, have achieved or are close to achieving the UNAIDS 95-95-95 targets: 95% of people living with HIV knowing their status, 95% of those diagnosed on treatment, and 95% of those on treatment virally suppressed. Achieving these targets globally would mean finally ending the epidemic as a public health threat.
Living with HIV Today
Thanks to decades of scientific progress, living with HIV today is radically different from the earliest years of the epidemic. Treatment is more tolerable, effective, and accessible than ever before. Individuals are living full lives, including starting families, maintaining long-term relationships, and contributing in every sector of society. Yet, disparities persist in access to care, affordability of drugs, and social acceptance—especially in resource-constrained settings and among marginalized groups.
Regular medical visits, adherence to medication, and healthy living practices remain essential. The long-term side effects of HIV or its treatment—such as cardiovascular disease, kidney disease, and malignancies—are now more relevant in an aging HIV-positive population. Still, with proper management, these can be mitigated, and quality of life remains high for most.

HIV/AIDS is a testament to the dual nature of human frailty and resilience. It is a disease that has both highlighted and challenged our medical knowledge, societal values, and collective will.
